What is the procedure for requesting a new patient appointment?
Please complete the PDF Fillable Prescreen Form found on the Contact Page;  save it to your computer and email it to  info@swc-nj.com

How do I request a refill for medication?
If you need a refill of your medication, you can send us a message through your OFFICE ALLY PORTAL, email us at info@swc-nj.com or call the office directly at 732-444-8802. Medications will be filled within 24 hours unless it is a weekend or holiday in which it will take up to 72 hours. We do not respond to faxed refill requests from your pharmacy.

What if I need external paperwork filled out?
Please contact our office to let us know what type of paperwork needs to be filled out. In certain circumstances an office visit may be required.

Does our office prescribe Emotional support animals?
No, we do not.

Why does Shrewsbury Wellness Center require a credit card on file?
Shrewsbury Wellness Center has a credit card on file policy. Co-insurance payments, deductibles, co-payments, No Show/Late Cancellation fees will be charged to this payment method unless another form is received at the time of the scheduled appointment or prior arrangements have been made with our office.
